Case Management
Community Bridges Case Management staff works with individuals and families in a collaborative planning process to determine what the choices and goals are for the future. A team is then established with the individual to implement, coordinate, monitor, and evaluate the options that have been chosen.

Community Bridges provides

  • Advocacy and assistance in problem solving and goal setting
  • Development of an Individual Support Plan for those 21 years and older to work toward these goals
  • Coordination of all aspects of the individual's plan among social service and other agencies
  • Assistance in obtaining whatever supports are needed, such as housing, work, transportation, home visits, home modifications and adaptive equipment referrals, and support groups
