Early Supports and Services
Early Supports and Services serves in the early identification, treatment and prevention of developmental delays for those infants and toddlers who have a delay in any area of development or are at risk for disabilities as determined by at least two professionals.

Community Bridges provides

  • Support and information to families regarding infant/toddler development
  • Home-based services for children with developmental delays
  • Comprehensive coordination of services and programs in which families can participate
  • Transition assistance to available community supports and services at age three
  • Consultative services to other community programs

Eligibility

Any infant or toddler who has a delay in any area of development or is at risk for disabilities as determined by at least two professionals is eligible for services.

Getting Started/Making a Referral

Anybody can make a referral to Community Bridges' Early Supports and Services Program-parent, relative, friend, pediatrician, childcare provider, member of the clergy. Most inquiries come from parents who are concerned that their child is not keeping pace with their peers, or just have a sense that something is not quite right. Parents know their own child's abilities and frustrations best.

When we receive a phone call asking about Early Supports and Services, we first explain the program's overriding philosophy that parents are the primary decision makers. We then explain the steps in the assessment process, starting with an initial meeting in the child's home to do an informal observation and to answer any questions.

Initial Assessment

A formal assessment is scheduled that typically takes two hours and takes place in the home-the most comfortable and familiar place for most children. This is a global assessment in all areas of development-motor, cognitive, language and social-that is play-based.

At the time of assessment, parents are informed of eligibility according to criteria set by the State of New Hampshire. Subsequent services are individually designed, based on discussions between the parents and the assessment team.

By working closely with families, we help parents address their concerns, prioritize their needs, explore their resources and set a course of action.

Individual Family Support Plan

This written plan of action is based on the assessment and is developed by the family, Community Bridges staff members and anyone else the family wishes to include. The plan contains the goals, activities and supports that the family has chosen. The plan reflects the unique needs and strengths of each family, and is intended to support families in the context of their lives and communities.

Our Early Supports and Services staff helps parents understand how children grow and develop, how to play and talk to their children, and how to handle their children's behavior. We also provide access to a parent support network.

Who Pays for These Services?

Early Supports and Services is a federal entitlement program. Families are not billed for services. We do pursue private insurance and Medicaid reimbursement, as required by law. At no time is any child denied services because of the family's inability to pay.

What Happens at Age Three?

As determined by state and federal law, Early Supports and Services are provided until the child's third birthday. At that point, the child's school district becomes responsible for his or her educational needs.

Community Bridges helps families identify and access the community supports and services that are most appropriate for their child. We help make these transitions smooth by serving as a link to appropriate community options.
