Eligibility for Services

Through our relationship with the State of New Hampshire as an area agency, Community Bridges opens doors to a wide range of supports for children and adults with disabilities. First we guide applicants through the steps for reviewing eligibility. These steps serve to answer two important questions:

  • Has a trained psychologist or psychiatrist worked with the person to complete a test that measures his or her ability to think and solve problems, and are those abilities significantly below others of the same age?
  • Does the person have severe difficulty in completing most of the basic activities involved at home and in the community?

 

 

The eligibility determination process is different for children three and under and for those over the age of three.

Who Can Make Application

  • Community members who know the child,
  • The applicant or the guardian of the applicant under the age of 18 Or
  • Guardian of the applicant age 18 or over who has been appointed by the probate court
What To Expect The Intake Coordinator will
  • Record your identifying information
  • Schedule an interview at your home to complete the application process
  • Collect any additional information and records needed to complete a comprehensive assessment for eligibility
  • Notify you in writing of your eligibility status
  • Place your name on an Awaiting Services Register with the State of New Hampshire if resources needed for your support are not immediately available

Who May Be Eligible

DEVELOPMENTAL SERVICES

Over the age of three, an individual may be eligible if there is a documented disability that occurred in the developmental period (before age 22) that is attributable to at least one of the following:

  • Mental Retardation
  • Cerebral Palsy
  • Epilepsy
  • Autism
  • Specific Learning Disability

Any other condition of an individual found to be closely related to mental retardation as it refers to general intellectual functioning or impairment in adaptive behavior or to require treatment similar to that required for individuals with mental retardation.

AQUIRED BRAIN DISORDER

An Acquired Brain Disorder is defined by the State of New Hampshire to mean; a disorder of the brain that is not congenital, presents a severe and lifelong disabling condition which significantly impairs an individual's ability to function in society, occurs after the age of 22 but prior to the age of 60, and is due to one or more of the following:

  • Physical Trauma as a result of a car, motorcycle, or bicycle accident, or other related physical accident or occurrence
  • Infectious disorder such as encephalitis or meningitis
  • Cerebral Vascular Disease such as stroke
  • Demyelinating or other inflammatory diseases such as multiple sclerosis
  • Toxic metabolic disorder such as anoxia
  • Other related neurological disorders such as Huntington's Disease
