Through our relationship with the State of New Hampshire as an area agency, Community Bridges opens doors to a wide range of supports for children and adults with disabilities. First we guide applicants through the steps for reviewing eligibility. These steps serve to answer two important questions:
- Has a trained psychologist or psychiatrist worked with the person to complete a test that measures his or her ability to think and solve problems, and are those abilities significantly below others of the same age?
- Does the person have severe difficulty in completing most of the basic activities involved at home and in the community?
The eligibility determination process is different for children three and under and for those over the age of three.
Infants and children under three
