 
 

| Family Support Council The Family Support Council is comprised of individuals who have a family member with a disability and reside in Merrimack County (excluding Hooksett) or the towns of Deering, Hillsboro, and Weare. Membership on the Family Support Council gives families an opportunity to work with other people who have a family member with a disability, and to network with other community agencies and organizations. Members share their experiences and their approaches, their ideas and their dreams. Membership benefits Community Bridges, and therefore families and individuals with disabilities. The Council works to improve the agency's planning process and helps Community Bridges implement it's mission. Becoming a member benefits you through opportunities to utilize your current skills and learn new ones. In addition to learning more about Community Bridges and the State Department of Developmental Services, you will discover new ways to have a positive impact on your loved one's life. The Council meets on the fourth Wednesday of every month from 6:00-8:00 PM at 525 Clinton Street in Bow. Partners in Health Council
All families with a child, birth to 21, with a chronic health condition (including blood disorders, cancer, cerebral palsy, diabetes, epilepsy, heart conditiions, muscular dystrophy, pulmonary disorders, & spina bifida) are invited to join the Partners in Health Council. The Council provides guidance and direction to Partners in Health. It empowers families and gives them a forum and resources for mutual support, to make decisions about their own children, and to engage in effective community outreach. The Council provides critical links in the ongoing work of helping parents make their needs, preferences and concerns known to the larger community.

Partners in Health recognizes the value of:
- Bringing together families who have children with chronic illnesses;
- Working with schools, medical professionals, churches, social services and other community organizations to facilitate and enhance care and services;

Providing families with an outlet for the stress of their daily routines by creating social and recreational opportunities with other families who share similar concerns.
The Partners in Health Council meets on the first Monday of every of month from 6:30-8:30 PM at 525 Clinton Street in Bow.
