| Beth: Transition from Middle School to High School During the elementary and middle school years, Beth had numerous opportunities to be fully included with all students in a variety of settings. She was part of a "typical" class, joined after-school sports activities and clubs, and enjoyed outings with peers.
Shortly after her freshman year in high school began, it became clear to Beth and her family that the mainly segregated "Life Room" model was not going to provide a satisfactory educational experience for her. For the first time in her life, Beth was separated from many of her friends and the classes that seemed most interesting to her were not offered to her. Even lunch in the cafeteria with other students did not seem to be an option. It became clear to Beth and her family that the school did not have a sense of Beth's personal vision of how her education could be designed in order to meet academic and personal goals.
Beth and her family decided to engage in person-centered future planning facilitated by Beth's Community Bridges Family Support Liaison. They invited several key school supporters to be involved in the process. The planning process enabled Beth and her supporters to clearly articulate Beth's current interests, strengths, talents, and support needs as well as identify some goals for the future that Beth would like to work toward.
