Partners in Health
Many families face the challenges of raising children with chronic health conditions such as cancer, diabetes or cerebral palsy. At times, the issues may be overwhelming for the entire family, and can interfere with the quality of family life. Often, relying on relatives and friends is simply not enough. Partners in Health supports families with chronically ill children with understanding, respect, and accommodations that address their needs within their own environment. We help families get the support and information they need to manage the chronic illness of their child without leaving their community. Partners in Health recognizes the value of: - bringing together families who have children with chronic illnesses
- working with schools, medical professionals, churches, social services and other community organizations to facilitate and enhance care and services
- providing families with an outlet for the stress of their daily routines by creating social and recreational opportunities with other families who share similar concerns
Eligibility Any family in the greater Concord area with children from birth to 21 who have a chronic health condition, including: - Blood Disorders
- Cancer
- Heart Conditions
- Pulmonary Disorders
- Diabetes
- Epilepsy
- Spina Bifida
- Muscular Dystrophy
- Cerebral Palsy
Anyone can make a referral to Community Bridges Partners in Health-parent, relative, friend, pediatrician, teacher, child care provider, member of the clergy. Community Bridges is the lead agency for Partners in Health in the Concord area and surrounding communities. The Partners in Health program represents a broadening of our early intervention and family support capacity to include children with chronic health issues. At Community Bridges, we create a partnership between families and professionals to develop individualized approaches that are flexible, resource-based and value the family's experience, strengths and insights. Partners in Health is administered by the Hood Center for Children & Families and the State Council for Children & Adolescents with Chronic Health Conditions. Partners in Health Family Support Coordinator assists families in - identifying needs
- accessing appropriate services and resources
- advocating to schools, insurance companies and medical providers
- arranging for special needs during hospitalizations and after discharge
