Community Bridges
Working Together to Support People
Community Partnership

Community Bridges acts as a bridge linking families and individuals with disabilities with area schools, healthcare providers, public agencies, businesses and human service organizations. We work hand in hand with provider agencies that offer a wide range of housing and support services. In addition to these day-to-day collaborations, Community Bridges is a lead agency in Partners in Health-a partnership with Dartmouth Hitchcock Medical Center to support families with children who have chronic health problems.

Community Support Network Inc. - CSNI

Community Bridges is a member of CSNI which is a not for profit organization that works in support of the 10 Area Agencies throughout the state of New Hampshire that provide services to individuals with developmental disabilities and their families. www.csni.org

Community Provider Network of Central New Hampshire - CPNCNH

We are also a member of the Community Provider Network of Central New Hampshire: a multi-agency coalition established to improve the health status and quality of life of the citizens of central NH. www.providernetwork.net/index.cfm

Partners in Health

Many families face the challenges of raising children with chronic health conditions such as cancer, diabetes or cerebral palsy. At times, the issues may be overwhelming for the entire family, and can interfere with the quality of family life. Often, relying on relatives and friends is simply not enough. Partners in Health supports families with chronically ill children with understanding, respect, and accommodations that address their needs within their own environment. We help families get the support and information they need to manage the chronic illness of their child without leaving their community. Partners in Health recognizes the value of:

  • bringing together families who have children with chronic illnesses
  • working with schools, medical professionals, churches, social services and other community organizations to facilitate and enhance care and services
  • providing families with an outlet for the stress of their daily routines by creating social and recreational opportunities with other families who share similar concerns
Eligibility Any family in the greater Concord area with children from birth to 21 who have a chronic health condition, including:
  • Blood Disorders
  • Cancer
  • Heart Conditions
  • Pulmonary Disorders
  • Diabetes
  • Epilepsy
  • Spina Bifida
  • Muscular Dystrophy
  • Cerebral Palsy

 

Anyone can make a referral to Community Bridges Partners in Health-parent, relative, friend, pediatrician, teacher, child care provider, member of the clergy. Community Bridges is the lead agency for Partners in Health in the Concord area and surrounding communities. The Partners in Health program represents a broadening of our early intervention and family support capacity to include children with chronic health issues. At Community Bridges, we create a partnership between families and professionals to develop individualized approaches that are flexible, resource-based and value the family's experience, strengths and insights. Partners in Health is administered by the Hood Center for Children & Families and the State Council for Children & Adolescents with Chronic Health Conditions. Partners in Health Family Support Coordinator assists families in

  • identifying needs
  • accessing appropriate services and resources
  • advocating to schools, insurance companies and medical providers
  • arranging for special needs during hospitalizations and after discharge
  • identifying needs
