Working Together to Support People
Direct Support Services
A specialized team of Community Bridges professionals-our Visions staff-works with adults with disabilities who need support and encouragement in creating and maintaining a home as well as in building positive experiences in the community.

Community Bridges provides

  • Specialized foster care arrangements where a family shares their home with someone who experiences a disability
  • Periodic scheduled staff support to someone living in an apartment to lend support in specific activities such as shopping or meal planning
  • Continuous care with staff available at all hours
  • Staff assistance where a person is employed
