Assistance Fund

Community Bridges is proud of the positive impact we make in the lives of the individuals and families we serve and in the community as a whole… BUT WE CAN’T DO IT WITHOUT YOU.

We continue to evolve service delivery and manage available resources to meet demands. Unfortunately, there are still needs our system cannot fulfill. Your contribution to the Community Bridges Assistance Fund enables us to help the individuals and families we serve overcome difficult financial obstacles when all other resources have been exhausted. For example:

Jane lives with a family in an Enhanced Family Care setting on the outskirts of Concord. She had dental problems that resulted in having all of her teeth extracted. She needed a full set of dentures after her gums healed but her Medicaid funding does not pay for dentures. NH Dental Society would pay for half the cost but Jane could only contribute $50 from her limited income.

After Stephen was involved in a motor vehicle accident, he was diagnosed with an acquired traumatic brain injury and a secondary seizure disorder. He lives in a residential treatment and rehabilitation program where he continues to make improvements. In order to reach his goal of becoming employed, Stephen needs the help of an employment specialist to become “job ready” but there are no funds available to pay for this.

This past summer, Lucy, a senior who is assisted by the Homecare Options Program called for help. After days of over 90º temperatures, she had to move out of her house and sleep on a friend’s couch for a few weeks because she could not afford an air conditioner. Through the Choices for Independence Waiver she can continue to live in her home but it does not provide for air conditioning or other everyday needs.

Your generous contribution to the Community Bridges Assistance Fund will help people like Jane, Stephen, and Lucy, and enable us to:

  • Purchase adaptive equipment for children to extend the therapies they receive at school to their homes.
  • Transport homebound elders to doctor’s appointments, community activities, or a friend’s house for a visit.
  • Supply low income families with children served by the Early Supports and Services program with developmentally appropriate toys and warm clothes.
  • Send children with disabilities to camp or after school activities with the support needed to continue socialization and make friends.
  • Provide families who can’t afford insurance access to preventative dental care.
  • Fill emergency prescriptions until the family’s medical benefits are approved.
