Community Bridges is dedicated to helping you stay in your home and remain an active member of your community. If you or your loved one is experiencing difficulty in carrying out tasks that once were simple but now create challenges in completing regular activities in your daily life, please give us a call. We have over 25 years of experience in community support, helping people who experience disability from a variety of causes.
CONSUMER DIRECTED PERSONAL CARE
This program enables seniors to remain in their homes, select their service provider,and manage how, and when they receive services.
Services may include assistance with:

Bathing & grooming

Dressing

Mobility
-
Housekeeping
-
Meal Preparation
-
Laundry
-
Transportation to shopping
-
Necessary errands and appointments

Self-administration of medications

Companionship/SafetyPaperwork and organization of records

Consumer Directed Personal Care Services gives you more control and more choices over your services and your life.
You may:

Choose your own Personal Care Services Provider (could be a friend, relative, or neighbor).

Advertise, interview, and choose your own Personal Care Services Provider from the directory of applicants

Interview a few individuals from our directory of pre-screened applicants and choose those you like best.

Or...have us select a Personal Care Services Provider that we think is a good match for you.
We then become the employer of record and handle all payroll issues and paperwork. We offer training and oversight of your provider.
YOU are in control of your services, your service provider, your schedule, and your personal care with OUR assistance!
Consumer Directed Services is a non-medical, hands-on service that can be paid for through the NH Medicaid "Choices for Independence" Program, your own private funds, or a combination of these sources.
