ASSISTANT MANAGER (Full-time)

Responsible for all day-to-day aspects of operation of a certified residence and/or other assigned service, ensuring that each person receiving services is provided supports as needed to develop and maintain one's independent living skills while preserving one's dignity and self-respect. Ensuring that DMHDS certification standards as well as agency policies and procedures are upheld throughout delivery of services. Will be responsible for the management of service operations and staff supervision of assigned location.  Bachelors degree in related field preferred.  At least 2 years working with people experiencing a disability, involving at least 1 year of supervisory experience of personnel and administrative duties.

IN HOME SUPPORT PROVIDER (Less than part-time, less than 19 hours/week)

Community Bridges is currently hiring staff for our In Home Supports Program.  We are seeking people who are committed to supporting individuals in reaching their highest potential.  As an In Home Support Provider you will be responsible for providing supports to individuals between the ages of 3-21, with guidance and oversight from a parent, both in the home and the community, as well as helping them to increase various skills.  A High School degree or equivalent is required.  Excellent communication skills and the ability to work independently are a must.  Experience working with individuals with disabilities is preferred but not required.  We are looking for a team player that believes in the value of diversity that represent the core principles of Community Bridges and the unique contribution that each person adds to the community.  Current opening in Chichester and Franklin.

OCCUPATIONAL THERAPIST (Full-time or Part-time)

Join our trans-disciplinary team! Busy Early Supports and Services program seeks an Occupational Therapist.  Program serves children living in greater Merrimack County, ages birth through three who have developmental delays and their families. Services are home and community based and family centered. Responsibilities include providing developmental assessments, therapy, case coordination, consultation, and providing family education and support. Bachelors or Masters in Occupational Therapy, experience with pediatric population, current professional license and a valid NH driver's license required.

PERSONAL CARE SERVICE PROVIDERS FOR ELDERS (Part-Time)

Seeking friendly, caring, responsible staff to assist people who are elderly or chronically ill, with homemaking services, companionship, dietary management, personal care and activities in the community.  A High School diploma or equivalent, clean criminal and driving records and reliable, insured vehicle are required.  We are recruiting for part-time openings in the Concord and surrounding areas.  Candidates with weekday daytime availability needed.  

PERSONAL SUPPORT PROVIDER  (Part-time and Per Diem)

Residential and Day Services offers a variety of employment options as a Personal Support Provider including community supports, in-home personal care, and residential care for adults.  These positions require a high degree of people skills that include good communication, dependability and a positive attitude.  We provide support to people with a variety of needs in a variety of settings.  Our support hours and days can include days, nights, weekends and holidays.  All of our positions, whether part-time or relief, require people with flexible schedules and a willingness to provide different types of support that can include:

·   Bathing & toileting assistance or total care

·   Assistance with grocery shopping

·   Driving a wheelchair van for medical appointments or community activities

·   Assisting or performing household tasks

In particular we are seeking people for the following situations:

PSP for several people in Merrimack County providing community access and assistance with activities, weekdays, may include driving wheelchair van or your vehicle. (Must have clean criminal and driving records & dependable, insured vehicle) 

PERSONAL SUPPORT PROVIDER - Forensic Program (Full-time, Part-time and Per Diem)

Seeking people to provide support in the Concord area to individuals experiencing developmental disabilities, who can exhibit challenging behaviors. Assist the individuals with independence building activities & social skills at home and in their community and learning appropriate social interactions and coping skills.

PERSONAL SUPPORT PROVIDER - Harrisville (Less than Part-time)

Seeking person to provide support to an individual experiencing a developmental disability in the Harrisville area. Assist the person with independence building activities & social skills in his home and community. 10 flexible hours on Fridays.

PHYSICAL THERAPIST  (Full-time or Part-time)

SERVICE COORDINATOR (Full-time)

We are looking for a dynamic person to work in support of families and individuals! We're seeking a career-minded, creative and hardworking person who understands the value and importance community connections and relationships have in everyone's lives. This position will support individuals who experience a developmental disability and their families to envision and make plans for experiencing a meaningful life.  Formal service coordination and facilitation of a person-centered planning process is an integral component of this role.  Applicants must possess strong communication and writing skills and have a good working knowledge of computer applications (Microsoft Office).  Excellent organizational and interpersonal skills are a must.  Knowledge of regional and state resources is a plus.  Bachelors degree plus 2 years of related experience preferred or an Associates degree plus additional relevant experience required.

Live-In Opportunities

HOME PROVIDERS

Seeking active, patient people or families in the Merrimack County and northern Hillsboro County area, open to inviting a person experiencing a disability to live with them in their home. Home Providers will help the individual to lead a successful, rewarding life by developing  relationships, participating in activities and becoming a positively known member of the community. Families involved in activities at home and in the community preferred. Some individuals may have a day program with staff, while others could be with the family throughout the day. Some accessible homes, some homes without young children and others without pets needed. Monthly stipend in return for support.

LIVE-IN COMPANION - Concord

Seeking a dedicated person to live in a private home in Concord with a person or people experiencing a disability. Must be available evenings and overnights, Monday-Thursday and all hours of the weekend, to support and teach the person basic living and self care skills, safety and recreational activities. Compensation includes free room.

LIVE-IN COMPANION - Franklin

Seeking a roommate for a home located in Franklin. Three adults experiencing disabilities currently reside in the home and have staff present 24 hours a day.  The new roommate is provided with free room and utilities in exchange for being available to assist staff with the individuals residing in the home, if there were an emergency during the overnight hours.  Candidates must be available 5 nights a week, hours can be flexible.  This opportunity may have small stipend, depending on the number of hours contracted.  As a roommate, this person will have their own bedroom and sitting room.  They may reside in the home 7 nights a week, treating it as their own home, or only be present 5 nights per week.  Houseguests are allowed.  Person willing to commit to at least 2 years preferred.  Candidates must have clean criminal and driving records.

Community Bridges supports individuals throughout Merrimack County (except Hooksett) plus the towns of Deering, Hillsboro, and Weare.
