HOME PROVIDERS

Seeking active, patient people or families in the Merrimack County and northern Hillsborough County area, open to inviting a person experiencing a disability to live with them in their home. Home Providers will help the individual to lead a successful, rewarding life by developing relationships, participating in activities, and becoming a positively-known member of the community. Families involved in activities at home and in the community preferred. Some individuals may have a day program with staff, while others could be with the family throughout the day. HOME PROVIDER - Concord

Immediate opening! Looking to place a young man who experiences a developmental disability into a home with two male adults, or one male adult who has access to a respite provider. Home Providers will provide a structured and inclusive home life outside of his staffed day program. Home Providers will help him with independent living skills, recreation and effective coping skills. Home Provider will attend team meetings and complete monthly paperwork. HOME PROVIDER - Merrimack County

Immediate opening! Share your home with a social young man who has a variety of leisure activities, enjoys the outdoors and is independent with his daily living skills. He requires support and guidance due to mild developmental disabilities. Home Providers will provide support as needed and be available evenings, overnights and weekends. Home Providers will attend team meetings and complete mandatory paperwork. Monthly stipend in exchange for support.
