Safe and Independent at Home
Seniors, adults with disabilities, and those with chronic illness have a great deal to share
with their communities, but may need some services to continue to live independent lives
in their own homes. Community Bridges Homecare Options Program (HOP) makes
this possible.
By receiving services at home, individuals continue to be involved, active members of
their communities. Many HOP participants continue to give back to their communities,
through volunteering, by providing much needed companionship to others, and by using
their life skills and expertise to further enrich the lives of those around them.
HOP is a non-medical, hands-on service that can be paid for through the NH Medicaid
"Choices for Independence" program, private funds, some long term care insurance
policies, or a combination of these sources.
HOP Program services include:
- Bathing and grooming
- Dressing
- Mobility
- Housekeeping
- Meal preparation
- Laundry
- Transportation to shopping, errands, and appointments
- Self-administration of medications
- Companionship and safety
- Paperwork and organization of records
Flexible and Supportive
All adults seeking services through HOP participate in an assessment, assist in
designing their own plan, and provide input when selecting their personal care service
provider. There are multiple options for selecting a provider and Community Bridges acts
as the employer of record, handling all of the payroll, paperwork, training, and oversight.
With support and assistance from HOP, the participant is in control of his or her own
services, provider, schedule, and personal care.
Homecare Options Program Specialty Services
Special Event Companion Service ($25/hr)
Have a graduation, wedding or other special event coming up? Need a companion for your older loved one who will be at the event just for them? Our Personal Care Service Providers will help your family member prepare for the event, provide transportation, and be their companion at the event. Our PCSP take the pressure off so you relax and have a great time.
Spring Cleaning Service ($23/hr)
Everyone can use a little help getting organized now and again, especially older family members. Take the stress out of spring cleaning by having an objective Personal Care Service Provider freshen up your loved ones home and even help them sort through their treasures.
Wellness Visit Service ($21/hr)
Wish you had more time to check in on your older Mom, Dad, or relative? No worries! Have one of our Personal Care Service Providers help you out by visiting for a couple hours. They will check the condition of the home, see if your loved one needs anything, and find out how things are going. After the visit you will be provided with a written report so you can stay updated.
Temporary Personal Care Service ($23/hr)
Have you recently had a family member return home from the hospital or from rehabilitation? Do you think they aren’t quite ready to care for themselves but you need to get back to work? We have Personal Care Service Providers who can help with bathing, dressing, meals, housekeeping and laundry until your loved one can care for them self again.
Menu and Meal Prep Service ($23/hr) ($0.46/mi)
Our Personal Care Service Providers can help with your loved one’s meals. They will visit your loved one, sit with them to plan a weekly menu, do the shopping, and prepare a weeks worth of convenient meals for their freezer.
Respite Services ($21/hr)
Going out to dinner or just need to get away but want make sure your older family member is safe, happy, and well cared for? Our Personal Care Service Providers can help! You will be able to relax and enjoy your time knowing that your loved one is being cared for.
Companionship Service ($21/hr)
Do you have an older family member that just needs some company? We can help! Our Personal Care Service Providers will spend time with them while they do their gardening, decorate for the holidays, or catch up on their letter writing. They will even help get some chores done while they visit!
Escorted Transportation Service ($21/hr) ($0.46/mi)
Could you use help getting your older family member to doctors appointments, hair appointments or out for other errands? Our Personal Care Service Providers are trained in Defensive Driving and can provide safe “escorted transportation” for your loved one.
