Community Bridges is a not-for-profit agency with a mission to advance the integration, growth and interdependence of people with disabilities within their own communities. In New Hampshire, we serve Merrimack County (except Hooksett) plus the towns of Deering, Hillsboro and Weare.

Community Bridges works with individuals with disabilities, their families and community members to:

• Identify the barriers that make participation in the community difficult  
• Look for creative and unique solutions to overcome those barriers  
• Work from people's strengths and abilities to help them achieve a good life  
• Create opportunities for people with disabilities to develop friendships and a network of relationships  
• Find ways for all people to give back to their community

Working in partnership with families and other community organizations, Community Bridges acts as a facilitator, change agent and broker of services. We provide a bridge between people with disabilities and the supports they need to maximize their potential.
