Community Bridges
Working Together to Support People
Programs

Think of Community Bridges as a broker, a facilitator, a focal point for action and support. We act as a bridge between people with disabilities (and their families) and a variety of community resources. We avoid saying that we "provide" services, because in most cases, the actual services are provided by one of our excellent provider agencies.

We serve our consumers through four major programs:

Program Serving Objectives
Early Supports and Services Children under 3 Identify, treat & prevent developmental delays
Family Support Services Families of children over 3 and adults Assist and empower families with information & referral, planning, training & advocacy
Service Coordination Adults over 21 Work with individuals and their families to create, implement and monitor their development plans
Direct Support Adults over 21 Provide support and encouragement to consumers in their home, work and community settings
Partners in Health Families of children ages birth-21 Provide families with support, information & resources
