THE SELF MANAGED SERVICE OPTION
Community Bridges Self Directed Services offer families the ability to design and manage their supports and services. What does Self Directed Services mean? It means YOU, the individual or family member, maintain optimum choice and control over your services by managing them yourself. As a Family Managed program through Self Directed Services you:
- interview and choose your staff including assuming responsibility for supervising, determining pay rates and incentives, and completing and submitting staff timesheets
- develop and monitor your budget and make decisions and adjustments as needed
- have first hand knowledge of your family member's progress toward goals
- can be creative and flexible with designing supports and scheduling staff to best meet your family member's needs
- complete all regulatory requirements including timesheets, attendance logs and progress notes
As a Family Managed Self Directed Services program you still retain access to an experienced Service Coordinator who can guide you through the process of program start-up and provide ongoing support and budget monitoring.
Your service Coordinator will:
- provide and review with you, the Self Directed Services Guidebook and Daily Operations Binder that outlines your responsibilities as a Self Directed program
- develop the annual Individual Service Plan and Consolidated Development Services Plan that drives the supports and services
- be available for discussion and support around recruiting, budget management and developing goals for your family member
- Insure that your budget dollars are spent wisely and in a manner that best meets your family member's need
