These positions support community members and their families who are connected to Participant Directed and Managed Services

When individuals and their families need to hire support staff, Community Bridges aids in recruiting, interviewing, setting expectations, and training. The Family Manager is responsible for providing supervision, approving hours worked, mileage, and monthly progress notes of the staff. The family approves invoices from contracted providers and for approved services, activities, and therapies as well. Community Bridges gladly provides this support and guidance, so your outcomes will be more successful.

We believe that people deserve to have choice and control over their Home and Community Based Medicaid Waiver funds that are made available for their use. Participant Directed and Managed Services assist you through workshops and individual guidance to maximize your own success. “Participant Directed and Managed Services” means just that! It provides the highest level of individual and family direction and decision-making with regard to formal service as well as flexibility in the utilization of funding.

While the benefits of this service structure may increase the responsibility that a family has, we believe many families are up to the task, which is why we’re here to support them!

Job responsibilities include after-school hours 1:1 in the family home to start. 1:1 support for recreational activities, playing, and all-round fun. A Fun-loving, charismatic 28-year-old male in Allenstown is seeking a Direct Support Professional to provide transportation to and from his place of employment and to provide assistance within the community. Hours: 15-20  $14/hour  Typical hours would be 8:30-11:45 (more available at times)

Full-Time Direct Support Professional
Rate of Pay:$16.00
Schedule (hours to work & days of the week): Tuesday-Saturday 11:00 am-5:00 pm

The ideal candidate for this Direct Support Professional is a responsible, patient, reliable, dependable, and creative person with a good sense of humor who is willing to take the initiative to help a fun-loving young man find new things to participate in. When the individual feels up to it, (no snowy days or bad weather days), staff will take the individual out in the community for 2-3 hours (vehicle provided).

Job responsibilities of the DSP include activities of daily living, including feeding, personal care, stretching, repositioning, playing games, helping him stay connected with the outside world via social media, and assisting him to make his own choices throughout the day using his assisted communication program.

Full training provided.

Consideration will be given to those looking for a part-time position. Part-time employees do not receive health benefits, although they do receive paid time off if hours are 20 or more.

Job Title:  SDS Direct Support Professional

Position Description:

A family in the Self-Directed Services (SDS) Program at Community Bridges is looking for a person on a part-time basis who will assist a quiet young woman with Down Syndrome. The position includes assisting this client to access her community for 20 hours a week in the Weare/Concord area. The ideal applicant must be a reliable, patient, and kind person who is willing to take the initiative to help this young woman find ways to achieve her goals, make plans to join peers in their common activities and achieve success at her volunteer job. The community's pre-planned activities would include bowling, the gym, outside activities that are season-appropriate (short hikes, basketball), shopping, and cooking simple meals. We would also be open to other ideas that are in line with her written goals.

The applicant must have his/her own vehicle and auto insurance in order to safely provide transportation to different locations in the community.
