Community Bridges offers a variety of support and services for individuals with developmental disabilities or acquired brain disorders and their families. Our goal is to help these individuals live their best lives throughout their lifespan.
If you believe you need support or services, please contact our Intake team to begin learning the steps and criteria to connect to the Area Agency system.
Our staff will assist you during the intake process to determine whether you or your loved one has a developmental disability or acquired brain disorder, as defined by New Hampshire statutes and rules. In New Hampshire, everyone has the right to request a complete intake assessment at an area agency. However, proof of a diagnosis for a developmental disability or acquired brain disorder is required.
We will help you understand the requirements for determining your eligibility and assist you in gathering the necessary documentation. During the intake discussions, we will also inform you about the range of supports and services available through the developmental services system and clarify any additional eligibility requirements for those services. Our aim is to support you in thinking in a person-centered way about what is important to you and your family, your priorities, choices and needs, and which supports would be most beneficial to you.
