False Claims Prevention and Whistleblowers Policy
This Policy serves to inform Community Bridges' contractors, agents, and employees of the provisions, penalties, and whistleblower protections associated with the Federal and State False Claims Acts, to remind contractors of our good faith efforts to prevent and detect false claims, and to remind employees and inform agents of our Compliance Program.
How to File a Complaint
We are required to provide information on the rights of clients receiving developmental and acquired brain disorder services in the State of New Hampshire to all the people we serve, both verbally and in writing. We do this annually at the Individual Service Plan meeting. We have provided three documents to assist you with familiarizing yourself with your rights. We have also included the process for making a complaint if you believe your/your family member’s rights have been violated.
Complaint Investigation Response to Recommendations
Providers must use this template to provide recommendations in response to a complaint.
New Hampshire’s Rights & Responsibilities Booklet For Individuals Receiving Developmental or Acquired Brain Disorder Services
This booklet guides those receiving services to understand their rights and responsibilities when engaging in services.
