Privacy Policy

We have created this statement to demonstrate our firm commitment to your privacy. We do not collect personally identifying information about you when you visit our site, unless you choose to provide such information to us. Providing such information is strictly voluntary. This policy is your guide to how we will handle information we learn about you from your visit to our website.

False Claims Prevention and Whistleblowers Policy

This Policy serves to inform Community Bridges' contractors, agents, and employees of the provisions, penalties, and whistleblower protections associated with the Federal and State False Claims Acts, to remind contractors of our good faith efforts to prevent and detect false claims, and to remind employees and inform agents of our Compliance Program.

How to File a Complaint

We are required to provide information on the rights of clients receiving developmental and acquired brain disorder services in the State of New Hampshire to all the people we serve, both verbally and in writing. We do this annually at the Individual Service Plan meeting. We have provided three documents to assist you with familiarizing yourself with your rights. We have also included the process for making a complaint if you believe your/your family member’s rights have been violated.

This booklet guides those receiving services to understand their rights and responsibilities when engaging in services.

How to File a Discrimination Complaint

If you have questions about your rights, please contact your Service Coordinator. They are prepared to answer your questions and provide clarification.  They can also assist you by filing a complaint on your behalf.
