Enhanced Family Care adopts a "shared life" approach to delivering quality care and support for individuals with disabilities. A vetted Home Care Provider welcomes the person needing support into their home, integrating them as an active member of their network of family, friends, and acquaintances. This approach effectively helps individuals with developmental disabilities learn life skills, build self-esteem, and lead more interdependent lives within their communities. We believe that inclusion and lasting relationships are essential for a fulfilling life.

Home Care Providers are actively involved in individual-centered conferences and team meetings, and they implement the objectives agreed upon by the support team for your loved one. Each Home Care Provider undergoes a thorough screening process, which includes interviews with the individual, their guardian, and the program manager. Selection is based on compatibility. All Home Care Providers receive comprehensive training and ongoing assistance from program staff and an Agency Nurse Trainer to ensure they meet the individual’s needs while adhering to all state and federal regulations.

 

Consider becoming a Home Care Provider

We are in search of an individual, couple, or family in central New Hampshire interested in becoming shared Home Care Providers.

When you become a Home Care Provider, you are not only opening your home to a person who will benefit from shared living, you are also making a positive impact on their life. You are helping build community connections and creating a supportive family atmosphere. Applicants must be compassionate, patient, and have a passion for enhancing the lives of those they support.

Community Bridges works with both the Home Care Providers and the individuals to find the right match for the best possible experience.

Responsibilities & Expectations:

Home Care Providers are expected to assist the person you support with an array of daily activities to include:

  • personal hygiene routines,
  • assist with or provide meals, and
  • assist with medication administration if needed.

The Home Care Provider is required to have a bedroom for the person and assume all responsibilities for the care of the home (i.e., lease, utilities, maintenance, etc.).

The selected Home Care Provider will receive:

  • training,
  • ongoing supports,
  • monthly stipends that cover operating expenses of the home and the expenses associated with caring for the individual.

It is vital that you are able to work well with the Individual’s support team.

Any previous experience working in health, the medical field, or as a shared Home Care Provider is preferred.
