Empowering Individuals with Disabilities
Community Choices is the direct service provider program for Community Bridges, which is dedicated to supporting individuals with disabilities or acquired brain disorders. Our organization aims to strengthen communities by empowering people with developmental disabilities and acquired brain disorders to remain active members of their community.
In partnership with those we serve and their support networks, we identify opportunities to create connections and build relationships through person-centered planning. This approach leverages each person’s skills and life experiences, enabling them to contribute to and thrive within the community.
We explore unique and creative ways for individuals with disabilities to enrich the lives of others in their communities, whether through employment, companionship, or offering support to those in need. Additionally, we identify barriers that may hinder individuals from staying in the community and help them focus on their strengths, interests, and abilities to overcome these challenges.
Residential and Community Engagement Programs
We empower everyone we support to participate in creating their life plans, making decisions, and contributing to their Community Choices partnerships. Our caring staff provides direct support to guide them toward living the best self-identified life possible.
Community Engagement Programs provide support services in natural, community-based settings. We tailor our care and services to meet your family’s unique needs while ensuring that you or your loved one feels like an integral and contributing member of the community. Our support can include:
- Personal care assistance and instruction
- Enhancing mobility and accessibility through modifications
- Connecting your loved one with the local community for socialization and peer support
- Teach personal management and budgeting skills
- Promoting a healthy and safe lifestyle
- Implementing person-centered planning so that every individual enjoys a dignified life
Our residential programs consist of nine homes, operated and staffed 24 hours a day by Community Choices. Five homes accommodate individuals who present behavioral challenges, and four homes support individuals with complex medical needs, aging issues, or acquired brain disorders.
Our Enhanced Family Care employs a "shared life" approach. Vetted Home Care Providers welcome individuals with disabilities into their homes, promoting active community involvement and personal growth. This method aids in developing life skills and self-esteem while fostering meaningful relationships.
