Family Support Services:
Ages 3 and Older
Family Support Services are provided to eligible individuals and their families as determined in accordance with He-M 503.
Every individual and family has unique values, beliefs, strengths, and capacities. They also have varying needs and preferences when it comes to selecting specific supports and services. Our family-directed approach encourages families to identify their unique priorities and needs, which helps determine the nature, focus, and scope of the supports they choose to receive.
We believe in a "person and family-centered" approach. This means:
- The strengths and capacities of the individual and their family are recognized and valued.
- Families are viewed as the experts regarding their family unit and the supported family member.
- The individual’s hopes, aspirations, and desired personal outcomes are the central focus of discussions and planning.
- Challenges and needs are addressed respectfully, with the intention of uncovering preferred and appreciated support and seeking creative, collaborative options for assistance.
- At every stage of life, individuals are encouraged to develop personal connections, explore opportunities that align with their interests and preferences, promote learning and self-discovery, and gain decision-making skills.
- Families are provided with information and learning opportunities to facilitate fully informed decision-making.
- Families are supported in developing their knowledge and skills and are encouraged to connect with other families to expand their support networks.
What supports are offered through Family Support Services?
The focus of Family Support Services can be flexible to meet your changing priorities needs over time, and to support you through life’s milestones. Some of the ways we can support you include:
- Family Respite
- Information about and referral to and connection with community resources and services
- Family to Family Connections
- Preparation for and attendance at school meetings as requested
- Planning for future services, exploring a variety of service provision and funding options
- Assistance with planning for life transitions, from early supports and services, to school services to adult life
- Education planning in collaboration with the school including High School transition to adult life
- Assistance with applying for state and federal benefits
- Assistance in accessing home modifications or adaptive equipment
- Referral to community funding sources for desired activities, equipment, emergency needs, etc.
- Trainings and workshops for individuals and families
- Community networking events for individuals and families
State regulation He-M 503.08 outlines the advocacy role of the Service Coordinator, including person-centered service planning, team meeting facilitation, service quality monitoring, documentation, reporting requirements, and other aspects. You can choose to have a Community Bridges Service Coordinator provide this role or consider alternative ways to receive this support. We can help you explore the various options for your Service Coordinator provisions.
