Welcome to Community Bridges.  A new diagnosis or learning about a new system of care can be a scary time, but it doesn’t have to be. At Community Bridges, we support individuals with developmental disabilities and acquired brain disorders, from birth through the lifespan.    We are here to assist you through every stage.  Whether you are joining us through our Early Supports and Services, our Family Support Program, or as an adult, we want you to feel secure in knowing that we are there to support you.

You may be looking for assistance with case management, special education, residential or day supports, or simply a listening ear - we are here for you.  Our staff is dedicated to ensuring that you receive the proper resources and referrals you need to make a smooth transition.  We are committed to ensuring that you are an integral part of your local community in every way.
