Direct Support Provider Floaters (Full-time) 

Seeking support staff to assist adults experiencing developmental disabilities with varying types of support, which may include: assisting with activities and goals such as making healthy food choices, grocery shopping, banking, budgeting, learning home safety skills, making social and community connections, learning household management skills; assisting with or performing household tasks; employment support, driving a wheelchair van for medical appointments or community activities; and providing support with personal care. Staff in these roles will float between our staffed residences for adults who experience developmental disabilities and higher medical needs and staffed residences for adults who experience developmental disabilities and may exhibit challenging behaviors. These positions require a high degree of people skills, dependability and a positive attitude. Medication Administration and MOAB trainings are required. Current needs include one first shift floater and one second shift floater.  Job Code: RDS Floater.

Direct Support Staff (Per Diem)

Openings for responsible, detail-oriented Support Providers with patience and good follow through needed to support adult females and males with developmental disabilities who have exhibited challenging behaviors.

Positive female and male role models preferred, to demonstrate appropriate behavior, provide consistency and supervision, and assist individuals with social skills, boundaries and goals. Staff will also assist with personal care needs. Previous professional experience surrounding behaviors preferred. Per Diem (Back up) staff needed for all shifts. Job Code: DSP BP Per Diem.

Direct Support Providers Day Programs (Part-time, Less than Part-time)

These positions require a high degree of people skills that include good communication, dependability and a positive attitude. All of our positions, require people with flexible schedules and a willingness to provide different types of support that can include: assisting in making healthy food choices, grocery shopping, banking, budgeting, leaning home safety skills, making social and community connections, learning household management skills, assisting with or performing household tasks, driving a wheelchair accessible van or personal vehicle for medical appointments or community activities and providing support with personal care. -Independent adult woman in Concord, Monday, Wednesday, Thursday & Friday, 5 hours/day, scheduled around individual’s appointments

-An adult woman residing in Chichester  Monday & Friday, 6 hours/day

-An adult female in Concord, 7.5 hours/week Saturday and/or Sunday.

-An adult female in Concord, 9 flexible hours/week Tuesday-Friday

-An adult male in Franklin, flexible mornings 6 hours/week

Direct Support Providers Residential Medically Fragile (Full-time)

Seeking Direct Support Provider for position that requires a high degree of people skills, dependability and a positive attitude. DSP must have a willingness to provide varying types of support which include: assisting with activities and goals such as making healthy food choices, grocery shopping, banking, budgeting, leaning home safety skills, making social and community connections, learning household management skills, assisting with or performing household tasks, driving a wheelchair accessible van or personal vehicle for medical appointments or community activities and providing support with personal care. Candidates must be comfortable with physical care with lifts and transfers and personal care. Passing Medication Certification class is required to maintain this position.  Current need is Monday- Friday 8:30-3:30. Job Code: Franklin.

Direct Support Providers Residential Medically Fragile Programs (Per Diem or Temporary)

Current Franklin/Northfield area needs are:

Temporary 40 hours per week, until Live-in Companion position filled, Sunday/Monday/Tuesday/Wednesday/ Thursday Asleep Overnight, 11pm-7am.

Temporary 32 hours per week, for about 3 months – Monday/Tuesday/Thursday/Saturday Awake overnight, 11pm-7am.

Per Diem all shifts.

Direct Support Provider Residential Male Behavioral Programs (Full-time)

Seeking responsible, caring people with high attention to detail and good follow through to support adult males who experience developmental disabilities. Individuals in this program have exhibited high risk, challenging behaviors. Positive male role models preferred, who can provide consistency, close supervision and redirection as needed. 

Assist the individuals in this program with learning appropriate social skills and boundaries, participating in recreational activities and building independence with activities such as cooking, cleaning, money management, personal care and finding or maintaining employment. Staff will be driving individuals to appointments, recreational activities and places of employment with their own vehicle. Previous experience supporting individuals with mental health issues or high risk behaviors required. Criminal Justice or professional legal system experience or education preferred! Candidates must have flexibility to work in different locations with a number of different individuals and abide by strictly upheld processes and protocols. Training is provided. Staff needed Tuesdays 3pm-11pm, Wednesday 7am-11pm, Thursday 7am-3pm and Friday 7am-3pm, in the Concord area. Job Code: BPM.

 Enhanced Family Care Program Manager (Full-time)

Community Bridges is seeking the right person to be responsible for all aspects of operation of individual services and/or other service in regard to Enhanced Family Care or Home Providers, ensuring that each person receiving services is provided supports as needed to develop and maintain one’s independent living skills, while preserving one’s dignity and self-respect. The Program Manager will ensure that DMHDS certification standards as well as agency policy and procedures are upheld throughout the delivery of services. They will be responsible for the management of service operations and Home Provider supervision. Candidates must have at least 2 years of experience working with people who have a disability, involving at least 1 year of supervisory experience of personnel and administrative duties. Bachelors degree or Associates plus 3 additional years of experience are required. Job Code: EFCPROGMGR.

In Home Support Provider (Less than part-time)

Community Bridges is currently hiring staff for our In home Support program in the towns of Allenstown, Concord, and Loudon. We are seeking staff who are committed to supporting individuals in reaching their highest potential. As an In Home Support Provider you will be responsible for providing supports to children and teens between the ages of 3-21, who experience a disability, with guidance and oversight from a parent, both in the home and the community, as well as helping them to increase various skills.

A high school degree or equivalent is required. Excellent communication skills and the ability to work independently are a must. Experience working with individuals with disabilities is preferred but not required. We are looking for a team player that believes in the value of diversity that represents the core principles of Community Bridges and the unique contribution that each person adds to the community. Job Coach (Full-time)

Our Job Coaches provide a range of work-related assistance including on-the-job supports to those with barriers to employment. Having sharp observation abilities, resume writing experience, problem solving capacity, and the ability to assess vocational abilities and safety skills are included as necessary skills for our challenging and rewarding position. These positions are community and sector based in the greater Concord, NH area. A High School Diploma or GED is required. Prior employment training preferred. Previous experience in the human service or related field with persons with disabilities is desirable.  Job Code: Job Coach.
