Person~Centered Transition Academy WHO: Designed for teams of high school students ages 16 – 20 WHEN: February 17th – May 25th (See session dates on next page) HOW: Virtual sessions through Zoom There will be “tracks” with sessions designed specifically for students, for families and for professionals, as well as sessions that bring Transition Academy participants all together for shared learning and connection. PRESENTED BY: Lesley Blecharczyk, Community Bridges Training & Program Development ~National Person Centered Thinking Trainer~ ~Charting the LifeCourse Ambassador and Coach~ Guide positive future planning, effective team communication, consideration of meaningful and creative experiences, and collaborative action planning Promote student ability to identify positive future life outcomes, and to guide and direct high school transition and adult life planning toward their “good life” Strengthen family ability to advocate on behalf of the student, identify their own desired future outcomes, and identify supports needed for success Provide common language and methods for communication, planning and problem solving that support individual goals as well as professional system requirements A pilot project that promotes and supports creative and collaborative transition planning with high school students, their family, school and professional supporters This project will introduce and explain how to apply Person Centered Thinking and Charting the LifeCourse concepts, skills, tools and practices that will: “The Learning Community for Person Centered Practices and Charting the LifeCourse Nexus support approaches through which people practice and learn together, and share experiences to cultivate vibrant learning communities. Central ideas will be shared with participants for application outside of session dates Participants will be provided with a workbook to support your own thinking and planning between sessions There will be an opportunity to receive individualized planning facilitation as part of what is offered through this pilot Sharing and collaborative learning from each other will be encouraged The hope is that this pilot group will offer feedback that will support on-going project demonstration across New Hampshire
