When a family member is diagnosed with a brain injury, developmental delay, or an age-related issue later in life, the news can feel overwhelming. Community Bridges was founded on the principle that people with disabilities are fully contributing members of our society and are integral to the success of vibrant, functioning communities.
We offer lifelong services to individuals with disabilities and their families, meeting your loved one where they are and offering encouragement and support through our network as they grow.
As part of our ongoing commitment to eligible individuals with a developmental disability or brain injury, we also offer short- and long-term person-centered planning, helping them prepare for a meaningful future. Working in conjunction with your family and alongside an individual’s personal network of service providers, we create an action plan to help them realize their personal goals.
